Home Health Aide/Certified Nursing AssistantTo be fully engaged in providing Quality/No Harm, Customer Experiences, and Stewardship by performing or assisting with basic patient care functions and providing appropriate age-specific therapy services in accordance with the plan of care established by the Case Manager.Adhere to all Health First, Inc., and Health First Home Care policies and procedures to ensure compliance with organizational and regulatory standards.Assist patients with personal care activities in alignment with the plan of care established by the Case Manager, ensuring compassionate and appropriate support.Perform HHA/CNA skills as directed and trained by the Case Manager to deliver high-quality care.Document services in real-time using Telephony, ensuring accurate and timely reporting.Ensure the safety of the patient and environment at all times, identifying and mitigating any potential risks.Notify the Case Manager and/or PI Supervisor promptly of any changes in patient condition and document changes as required.Support the orientation process for new associates, contributing to a knowledgeable and prepared team.Comply with all relevant State and Federal regulations and JCAHO standards to uphold the quality and safety of Home Care services.Actively participate in Quality Improvement processes, contributing to ongoing enhancements in patient care.Accept assignments and adapt to schedule changes, including weekend rotations and variable days off, to support coverage needs.Attend staff meetings regularly, maintaining at least 80% attendance to stay informed and engaged with team updates.Maintain 12 CEU credits annually for CNA certification and renew the CNA certificate promptly in accordance with established guidelines.Education: Highschool Diploma or equivalent.Work Experience: NoneLicensure: Current Certified Nursing Assistant (CNA) license in the State of Florida.Valid Drivers License and clean Motor Vehicle Record and maintained.Certification: Completion of a Home Health Aide (HHA) Seventy-Five (75) hour training course and Home Health Aide Certification (HHA).American Heart Association (AHA) Basic Life Support (BLS) Healthcare Provider Completion Card prior to start date and maintained.Background Check: Must satisfy Florida of Agency for Health Care Administration (AHCA) Level 2 background screening.Skills/Knowledge/Abilities: Good communication skills. Ability to interact with other professionals, families, and patients. Ability to work in stressful environments and/or situations. Ability to respond appropriately to life threatening situations. Ability to work independently with Telephony Documentation within time constraints and concentrate under multiple demands.Preferred Qualifications: Previous experience as a Home Health Aide (HHA).Physical Requirements: Ability to move in crowded conditions. Ability to operate automobile in variable conditions. Visual acuity and hand-eye coordination to perform clinical tasks. Must be able to lift, turn, pull, or ambulate with patients up to twenty (20) times daily. Must be able to lift up to forty (40) pounds unassisted up to six (6) times daily.
