This role ensures continuity of care when primary teams are unavailable, responding to urgent needs, managing symptoms, supporting families, and facilitating safe transitions of care.OverviewThe Hospice RN – After Hours delivers skilled nursing assessment, intervention, and coordination for hospice patients outside traditional business hours. This role manages urgent nursing visits, symptom escalation, after-hours admissions when indicated, and emotional support for patients and families, in alignment with hospice regulations, Medicare Conditions of Participation, and organizational standards.Key ResponsibilitiesRespond to patient and family calls and perform in-home or facility visits based on clinical necessityAssess symptoms and provide nursing interventions to promote comfort, stability, and safetyCommunicate with on-call providers to obtain medication orders and coordinate appropriate careRespond to patient death and complete nursing responsibilities including death pronouncement and post-mortem support within scopeCollaborate with emergency department providers and facility staff to support hospice-appropriate discharge plans aligned with patient and family goals of careConduct evaluation, consent, and admission visits when clinically indicated to ensure timely initiation of hospice services after hoursEnsure thorough clinical handoff and communication with the primary team following after-hours interventionsDocument assessments, interventions, and communications accurately and in real time in the electronic medical recordMaintain availability during assigned shifts in alignment with the on-call structureEnsure documentation supports regulatory, quality, and billing compliance requirementsEscalate care needs to the medical director or covering provider as clinically indicatedPerform other duties as assignedQualificationsGraduate of an accredited nursing program; Associate's degree or higher preferredCurrent RN license in the state of practice or valid multistate compact licenseMinimum of one (1) year of hospice, palliative care, or home health nursing experience preferredStrong clinical judgment, triage, and documentation skillsAbility to work independently and make sound clinical decisions in urgent situationsKnowledge of hospice regulations and Medicare Conditions of ParticipationCPR certification and valid driver's license requiredCommitment to delivering high-quality, person-centered care aligned with Arc Hospice & Palliative Care's mission and valuesThe starting compensation for this position is shown below.
