This role collaborates with program leadership and staff to support person-centered planning, ensure ISP compliance, maintain documentation standards, and promote quality service delivery.\nThe Program Specialist serves as a resource on ISP processes, participates in quality assurance activities, and contributes to Positive Behavioral Supports (PBS) initiatives across the organization.\nMajor Responsibilities\n\nISP Coordination & Monitoring\nMonitor ISP activities within the Home and Community Services Information System (HCSIS).\nReview and respond to alerts related to ISP deadlines and requirements.\nTrack documentation timelines and submission requirements.\nCommunicate HCSIS-related concerns and compliance issues to leadership.\nISP Planning & Facilitation\nSchedule and facilitate ISP planning meetings with program staff and stakeholders.\nAssist teams in identifying meaningful, measurable goals and objectives aligned with individual needs and preferences.\nSupport the development of data collection methods used to monitor progress toward ISP outcomes.\nReview progress toward established objectives and assist in updating plans as needed.\nTrack and coordinate ISP documentation throughout the planning and approval process.\nDocumentation & Quality Review\nConduct semi-annual reviews of ISP-related documentation and records.\nProvide feedback and guidance to managers regarding compliance and documentation standards.\nPrepare reports summarizing ISP documentation status, trends, and areas for improvement.\nSupport organizational quality assurance efforts related to ISP implementation.\nSatisfaction Surveys\nConduct satisfaction surveys with individuals receiving services prior to ISP meetings.\nCoordinate survey completion for individuals who require communication supports.\nCollect and manage annual family and guardian satisfaction surveys.\nConduct follow-up discussions regarding concerns identified through survey responses.\nTraining & Development\nAssist in training managers and staff on ISP requirements, best practices, and compliance expectations.\nSupport the development and delivery of ISP-related training materials.\nParticipate in the design and implementation of Positive Behavioral Supports (PBS) training initiatives.\nServe as a member of the Positive Behavioral Supports Steering Committee.\nAdditional Duties\nPerform other related duties as assigned.\nQualifications\n\nRequired Education & Experience\nBachelor's degree required.\nMinimum of two (2) years of experience working with individuals with intellectual and developmental disabilities (IDD).\nKnowledge of DDS ISP systems, processes, and regulatory requirements.\nProficiency in Microsoft Office Suite and related computer applications.\nValid driver's license with the ability to travel as required.\nKnowledge, Skills & Abilities\nStrong organizational, planning, and time management skills.\nExcellent verbal and written communication skills.\nAbility to analyze information and prepare accurate reports.\nAbility to facilitate meetings and collaborate effectively with multiple stakeholders.\nAbility to communicate effectively with individuals who have a wide range of communication styles and support needs.\nCommitment to person-centered practices and quality service delivery.\nWorking Conditions & Physical Requirements\nPrimarily office-based work with occasional travel to program sites and meetings.\nFlexible scheduling may be required to accommodate individuals served and family participation in interviews or planning meetings.\nMay be exposed to varying environmental conditions when visiting community or residential settings.\nExerts up to 10 pounds of force occasionally.\nPrimarily involves sitting for extended periods while performing administrative and computer-based tasks.\n\nAbout Company:\nApis Services, Inc.
