Decatur Wellness & Rehab is a 64-bed skilled nursing facility offering both short-term and long-term therapy services.
Join a supportive in-house rehab team dedicated to exceptional patient outcomes and meaningful collaboration.
️ Responsibilities
Review available resident information and evaluate its impact on assessment and treatment.
Conduct and document comprehensive functional assessments using standardized procedures.
Develop and implement individualized treatment plans with achievable functional goals.
Comply with evaluation, treatment, and documentation guidelines; maintain accurate records.
Enter treatment and progress notes after each session.
Revise treatment plans as needed based on resident progress.
Document treatment course in clear, concise discharge summaries.
Instruct Assistants in treatment procedures and provide close supervision.
Orient residents to program services and treatment procedures appropriate to their needs.
Collaborate with nursing staff, residents, and families to maximize functional abilities.
Instruct residents and caregivers on home exercises and therapeutic procedures.
Assist in referral processes for additional services following discharge; maintain knowledge of community resources.
Participate in rehabilitation team conferences, facility meetings, and quality improvement activities.
Access continuing education opportunities to maintain clinical competency.
Adhere to professional, regulatory, and facility standards.
Maintain expected productivity levels while demonstrating teamwork and strong clinical judgment.

✔️ Qualifications
Bachelor’s or Master’s degree in Physical Therapy
Active Tennessee PT license
Current BLS/CPR certification
