The LPN assists in planning, coordinating, and managing medical care services while promoting a safe and person-centered environment. The ideal candidate brings a can-do attitude, clinical knowledge, and compassionate support to individuals with developmental disabilities.Essential Responsibilities and Duties:Medical and Clinical Support:Collaborate with the Registered Nurse and healthcare team to implement and evaluate nursing care plans.Assist in coordinating total patient care in alignment with physician orders and agency policies.Monitor, document, and report changes in an individual's condition.Complete and maintain accurate clinical notes and progress summaries.Provide direct medical support including medication administration and basic treatments.Assist patients with activities of daily living while promoting independence.Maintain a clean and safe environment for individuals served.Respond to emergencies in accordance with agency protocols.Support physicians and Registered Nurses in complex nursing situations.Follow universal precautions and proper infection control practices at all times.Program Development & Monitoring:Serve as an active member of the Interdisciplinary Team (IDT), attending meetings and collaborating in the care planning process.Conduct and document medical assessments and health reviews as assigned.Develop, implement, and evaluate individualized healthcare programs and protocols.Gather and interpret data to monitor care outcomes and program effectiveness.Staff Support & Supervision:Participate in orientation and training of Direct Support Professionals (DSPs) in health-related procedures.Provide support and guidance to staff regarding medical care, including problem-solving and performance feedback in a timely, constructive manner.Recognize staff strengths and offer ongoing positive feedback to promote teamwork and development.General Work Behaviors:Embody and promote ABLE, Inc.'s mission, values, and core virtues.Work cooperatively across teams, communicating respectfully and effectively.Comply with all federal, state, local, and ABLE, Inc. regulations and policies.Maintain confidentiality and protect the dignity of individuals served.Be flexible and adaptable to meet the changing needs of individuals, teams, and the organization.Other:All other duties as requested or assigned.RequirementsQualifications & Skills:Required:Graduate of an accredited Licensed Practical Nursing program.Current LPN license in the State of Minnesota.Minimum of 1 year of LPN experience in a home care, hospital, or related healthcare setting.Valid CPR certification.Valid driver's license and reliable transportation.Strong interpersonal and communication skills.Ability to work independently and as part of a team.Flexibility to work varying hours and travel locally as needed.Physical Demands:The physical demands described here are representative of those that must be met by an employee to perform the essential functions of this role successfully.
