As a Home Health Concierge, you'll provide personalized, non-clinical support to participants-often one-on-one-in their homes or in care settings such as assisted living or skilled nursing facilities. Guided by the interdisciplinary team, you'll focus on enhancing comfort, independence, and overall well-being through meaningful, participant-centered support. If you enjoy building connections and making a daily difference in someone's life, this role offers a rewarding and impactful experience.Key ResponsibilitiesProvide Hands-On Care - Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting.Support Mealtime- Help participants enjoy their meals and stay hydrated for good health.Keep Things Moving - Support mobility, including turning, positioning, and safe transfers.Perform Vital Checks - Measure and record vital signs, weight, and intake/output.Maintain Comfort & Care - Keep rooms tidy, cozy, and well-stocked with essentials.Provide Responsive Support - Answer call lights quickly and respond with kindness and compassion.Monitor with Watchful Eyes - Report any changes in conditions to the leadership ASAP.Demonstrate Respect & Professionalism - Maintain confidentiality while delivering top-notch care.The above is a summary of the position, it in no way states or implies that these are the only duties this position will be required to perform.
