SENIOR MANAGER CARE COORDINATION (LICSW) – Senior LivingNeedham, MA | North HillLead care coordination for a vibrant senior living community and make a meaningful difference in the lives of older adults.For more than 40 years, North Hill, a not-for-profit senior living community in Needham, MA, has provided exceptional living and care for adults 65+. We are seeking a Senior Manager of Care Coordination to oversee Social Services and resident care coordination within our Independent and Enhanced Independent Living community. In this leadership role, you will guide resident transitions across North Hill’s continuum of care, collaborate with interdisciplinary teams, and support residents and families navigating complex care decisions. You will also mentor care coordination staff and help ensure a seamless and compassionate resident experience.Key ResponsibilitiesLead resident care transitions across the continuum of careOversee Social Services and Case Management for Independent and Enhanced Independent LivingChair the Care Coordination Committee and review at-risk residentsMonitor care utilization and service referral trendsSupervise and mentor care coordination staffSupport residents and families navigating care transitions and life care contractsQualificationsMaster’s Degree in Social WorkMassachusetts LICSW required3+ years of social work experience (senior living, healthcare, or CCRC preferred)Experience working with geriatric populations and interdisciplinary teamsNorth Hill OffersCompetitive salary and comprehensive benefitsHealth & Dental Insurance401(k) with company matchTuition Reimbursement & Career DevelopmentFree meals, parking, and on-site fitness centerJoin a mission-driven community dedicated to enhancing the lives of older adults.Pay Range: $120,000-$135,000/yearlyIndividual compensation within this range is based on a candidate’s specific qualifications, including education, overall experience, relevant experience, and specific skills.
