Registered Nurse - Home Health & Hospice

Position Overview: The Home Health RN provides skilled nursing care to patients in their homes across multiple counties. This role focuses on delivering high-quality, patient-centered home health services, supporting patients with ongoing medical needs, and ensuring safe, coordinated care while working independently in the field. Schedule & Work Details:Weekly Hour Guarantee: 40 hoursSchedule: M-F 8:00 AM - 4:30PMOn Call and Weekend Requirement: On Call 2-3 times per week and Weekend On Call up to every other weekendCoverage Area: Mercer Co, McDowell Co, Summers Co, Monroe CoMileage Reimbursement: Yes, reimbursed between patient visits from first patient to last patientPay Rate: $50-53 per hour Key Responsibilities:Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionalsSupervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulationsProvides required supervisory visitsProvides high-quality clinical services within the scope of practice and infection control standards, in accordance with the plan of care, and in coordination with other health care team membersCompletes comprehensive assessments (OASIS) including medication reconciliation accurately and timelyDocuments patient visits per policy and payer requirementMakes initial and/or comprehensive nursing evaluation visits, ensures patients meet home health eligibility and medical necessity guidelines, determines primary focus of care, develops the plan of care within State guidelines with the physician, and submits accurate documentationCommunicates relevant information timely and effectively with appropriate agency staff, including patient care issues, visit assignments, schedule changes, orders, OASIS data sets, coding requests, and coordination with other cliniciansCommunicates timely and effectively with physicians, patients, and family members to ensure quality care and service excellence Required Qualifications:Active RN licenseCurrent CPR certificationAbility to work independently in a home health environmentHCHB/PointClick Care experience Nice to Have QualificationsAdvanced vascular access management, including PICC line care and Port-A-Cath access/de-accessIndwelling and suprapubic Foley catheter insertion, maintenance, and troubleshootingVenipuncture and lab specimen collectionComprehensive wound care management, including complex dressing changes and negative pressure wound therapy (Wound Vacs)Tracheostomy care, suctioning, and patient/caregiver instructionOstomy care and education, including appliance selection, skin protection, and troubleshootingStrong infection prevention and control practices across all lines, wounds, and devices Pride-Health offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.
