Department Bariatric Ctr of Excellence Job Summary Under minimum supervision coordinates care of surgical bariatric patients from preoperative evaluation and education, through clinic care, and postoperative follow-up. Performs nursing care at an expert level, advising and monitoring delivery of patient care. Essential Functions: Demonstrates knowledge and applicability of the principles of growth and development over the life span, as well as demonstrating the ability to assess data reflecting the patient’s status and interpreting appropriate information relative to the age specific needs of the patient(s) to whom care/services is being delivered/provided. (10%) At a minimum, the success with which employees produce the results expected utilizing the above methods, techniques and approaches with specific patient age groups, shall be assessed via observation and/or competency review and/or patient outcome assessment. (10%) Performs according to the AHA Standards of Nursing Specialty. (10%) Knowledgeable in operational aspects of care throughout the care continuum including auxiliary departments (radiology, lab's, etc.). (10%) Is knowledgeable about the bariatric continuum of care including standards (national and regional), guidelines, and relevant policies and procedures. Maintains Bariatric Program database to include data-entry, reporting and troubleshooting. Ensures data is accurate entered in a timely fashion. (10%) Organizes and conducts bariatric patient group education sessions (English and Spanish) to include securing a room and speakers, preparing a power point presentation and developing and preparing materials for the session. (10%) Organizes the development of a comprehensive patient reference manual. Ensures manuals are attractive, translated, at the appropriate reading level and kept current (10%) Tracks and manages bariatric patient population. Ensures patients meet preoperative requirements and attend preoperative appointments. Ensures patients are not lost to follow up and receive the appropriate post operative care. (10%) Program marketing, promotional material development, web-site development, etc. Ensure materials and web-site remain up-to-date. (5%) Respond and provide any requested information to patients or referring providers by phone, e-mail and in person. Respond to requests for information generated from the DHMC website. (5%) Takes the lead in identifying Program needs and follows through. (5%) Participates in research activities. (5%) Education: High School Diploma or GED Required Work Experience: 1-3 years Three years of professional nursing experience.
