MDS Coordinator

Belvedere Nursing and Rehab in beautiful Hot Springs, Arkansas is needing you to join our team as an MDS Coordinator. You will be managing the MDS process for residents, and coordinate the care and rehabilitation services provide by the facility to residents, as directed by the treating physician. You must be either an LPN or RN in good standing and currently licensed by the state. Please come by 2600 Park Ave. in Hot Springs to meet us and complete an application. We cannot wait to work with you!! Job duties include: Conduct daily assessments of Medicare and managed care residents. Conduct weekly Medicare/PDPM meetings to review plan of care. Monitor MDS documentation, charting requirements and charting requirements that support services provided to meet billing requirements Chart MDS and pertinent narrative date to optimize case mix scores. Conduct daily assessment of Medicare residents and coordinate therapy, care planning and discharge planning as directed by the treating physician. Esure Medicare admissions are completed timely, included but not limited to: Ensuring orders are processed accurately and medications ordered and available in facility, immediate care plan completed within 24 hours, and risk assessments are completed. Coordinating the ARD with IDT to best maximize resident care levels. Daily morning rounds on Medicare and managed care and critically ill residents Attent scheduled care plan meetings for medicare and managed care residents. Monitor shift documentation on medicare and managed care residents, ensuring accuracy and timeliness of documentation. any other duties as assigned.
