We are creating a new version and looking for support setting up a survey in PaperSurvey.io so that we can collect data with which to examine the accuracy and consistency of our new tool. We have a survey and can get it uploaded into PaperSurvey.io. We are struggling to set it up to print the same code on three different documents (the new tool, a staff survey, and a youth survey) for 35 different groups associated with one of six different partner organizations nationwide.\r\nFounded in 1998, the Forum for Youth Investment is a trusted nonprofit leader dedicated to ensuring that all young people—and the adults who support them—have what they need to thrive. Our mission is to strengthen opportunities and outcomes for young people through continuous quality improvement, strategic partnerships, and policy. This project is critical to our efforts to support continuous quality improvement. The goal is to offer researchers and practitioners a short simple tool with very strong technical properties in order to better demonstrate the extent to which program quality impacts youth outcomes in out-of-school time organizations.\r\nWe have the tool and surveys completed and have practiced getting them uploaded to PaperSurvey.io.
