Join a team dedicated to clinical excellence and patient-centered care.PHYSICAL THERAPIST (PT)Summary: Performs patient-centered evaluations and treatments that meet all professional and regulatory standards.Essential Duties:Evaluation: Review physician referrals and medical records; perform comprehensive evaluations of strength, motor development, and functional capacity.Plan of Care: Develop and revise treatment plans to restore function, relieve pain, and prevent disability.Collaboration: Work with physicians and the interdisciplinary team to integrate therapy into the patient’s total healthcare plan.Supervision: Oversee PTAs, students, and unlicensed personnel per state and federal requirements.Documentation: Maintain accurate, timely patient records.Education: Guide patients on post-op recovery, physician protocols, and home exercise programs.Qualifications:Graduate of an accredited Physical Therapy program.Current or pending Ohio State PT licensure.AHA BLS certification (required within 30 days of hire).Experience in an outpatient therapy setting preferred.PHYSICAL THERAPIST ASSISTANT (PTA)Summary: Performs patient-centered treatments under the direction of a Physical Therapist and according to the established plan of care.Essential Duties:Treatment: Administer physical therapy treatments to restore function and relieve pain following the PT’s established plan.Communication: Regularly confer with the supervising PT and care team regarding patient progress.Documentation: Complete clinical documentation accurately and within facility timeframes.Education: Instruct patients and families on treatment plans and home exercise programs.Professionalism: Maintain flexibility in scheduling and participate in facility training and in-services.Qualifications:Graduate of an accredited Physical Therapist Assistant program.Active Ohio State PTA licensure.Current AHA BLS certification.Experience in an outpatient therapy setting preferred.Schedule & BenefitsStatus: PRN (As Needed).Hours: Daytime hours; no late nights, evenings, or weekends.Apply today at www.southwoodshealth.com#SWH
