Role & Responsibilities Assist patients with psychosocial assessments, develop care plans, provide counseling, and coordinate resources to support patient well-being. Collaborate with healthcare teams to ensure comprehensive care and advocate for patient needs. Maintain accurate documentation and adhere to clinical protocols. Licensed Social Worker Minimum of one year of recent clinical experience as a Licensed Social Worker. Valid BLS (AHA) certification required. Strong communication, assessment, and interpersonal skills essential for effective patient support and teamwork. Key Duties Conduct psychosocial evaluations, develop and implement care strategies, facilitate patient and family counseling, and ensure compliance with healthcare regulations.
