You will play a central role in supporting the care management team, managing referrals and enrollment workflows, coordinating with Managed Care Plans, and helping drive continuous improvement across all aspects of the program.\nThis position is primarily based at our corporate office in Moorpark, with regular travel 1-2 days per week to our San Fernando office to support the team based there. Frequency may vary based on team needs. On-site presence is required to support daily supervision, team collaboration, in-person training, and program oversight activities. Candidates must be willing and able to commute between both office locations. We are looking for someone who is compassionate, dependable, organized, and able to lead by example in a fast-paced program environment.\nCompensation\n$34.00/hour\nPosition Responsibilities\nSupervise Lead Care Managers, providing guidance and performance oversight\nOversee day-to-day operations of the ECM and CS programs to ensure continued performance and compliance\nMonitor caseload distribution, capacity, and workflow across the care management team\nManage referral, enrollment, and disenrollment processes across contracted Managed Care Plans\nMonitor Community Supports referrals and delivery of authorized services\nCoordinate with Managed Care Plan representatives on program requirements, reporting, and issue resolution\nSupport the implementation, updates, and adherence to program policies and procedures\nTrack program key performance indicators and prepare required reports and data submissions\nConduct quality assurance reviews of care management documentation, including MDT/ICT meeting notes, assessments, and care plans\nSupport audit preparation and respond to Managed Care Plan audit requests\nSupport onboarding and ongoing training of care management team members\nMay carry a small caseload of ECM members on an as-needed basis to support team capacity\nEmployees will be expected to travel to assigned locations as needed, and mileage will be reimbursed in accordance with company policy.\nRequirements\nMinimum 2 years of experience in Enhanced Care Management, Medi-Cal Managed Care, or a related care coordination program required\nFamiliarity with CalAIM and Managed Care Plan ECM/CS Provider Manuals required\nPrior supervisory or team lead experience preferred\nBachelor's or Master's degree in one of the following preferred:\n? Social Work\n? Psychology\n? Counseling\n? Rehabilitation\n? Gerontology\n? Sociology\n?
