As we continue to grow across Western New York, we're looking for a Bilingual Community Health Worker (Spanish/English) to support patients and families in Genesee County.This is a hands-on, community-focused role where you'll serve as a trusted link between patients, providers, and essential resources-helping individuals navigate the healthcare system while addressing real-life barriers to care. From supporting our local agricultural population to connecting families with critical services, your work will directly improve health outcomes and strengthen our communities.What You'll DoIn this role, you'll combine patient support, care coordination, and community outreach to:Help patients navigate the healthcare system, including scheduling, confirming, and coordinating appointmentsConduct needs assessments and identify barriers such as transportation, language, literacy, and financial challengesProvide Spanish interpretation and written translation within your scopeConnect patients to community resources like food, housing, insurance, and medication assistanceCoordinate transportation and other access-related servicesEducate patients on health promotion and disease preventionSupport outreach efforts and build relationships within the communityCollaborate closely with clinical and care teams to ensure coordinated, patient-centered careWhat We're Looking ForHigh school diploma or equivalentBilingual Spanish/English required (critical to supporting our patient population)Community Health Worker (CHW) certification or willingness to obtainExperience in community health, social services, healthcare, or outreach preferredAbility to support a wide range of responsibilities-from patient navigation and scheduling to case management, follow-up, and community engagementStrong interpersonal, communication, and organizational skillsComfort working with diverse populations while maintaining professionalism and boundariesWhy Join Oak Orchard Health?At Oak Orchard Health, we are committed to expanding access to care across Western New York-especially for underserved and agricultural communities.
