Social Determinants of Health Care Coordinator
Supporting the Social Determinants of Health screening and referral programs, the full-time remote SDOH Care Coordinator will manage data entry, place e-referrals, and follow up with families to connect Medicaid members with community resources across six states.
Key responsibilities
Enter SDOH assessment results into the Findhelp platform and verify patient Medicaid eligibility
Place e-referrals to community-based organizations and follow up with families to confirm referral outcomes
Coordinate with front desk staff to support screening workflows and contribute to continuous improvement efforts
Required qualifications
Bachelor's degree in social work, public health, community health, human services, or a related field; OR certification as a Community Health Worker (CHW) with relevant professional experience
Minimum of 2 years of experience in care coordination, community health, or social services with direct family engagement
Demonstrated understanding of social determinants of health frameworks and their impact on pediatric populations
Proficiency with web-based platforms and data entry, with a comfort in learning new technology tools
Ability to work independently in a fully remote setting, managing priorities across multiple states and time zones
