Working collaboratively with the San Francisco AIDS Foundation (SFAF), the San Francisco Department of Public Health (SFDPH), and community partners, the Coordinator ensures that participants receive high-quality, trauma-informed, harm reduction-focused services in a welcoming and culturally responsive environment.\nThe Coordinator oversees program logistics, participant engagement, incentive management, data collection, reporting, and collaboration with clinical partners to ensure successful implementation and positive participant outcomes. The program is designed to provide three 12-person cohorts annually, combining Spanish-language group sessions, individual support, clinical services, and incentive-based engagement.\nEssential Duties and Responsibilities\nProgram Coordination\nCoordinate all aspects of the Contingency Management Program.\nManage participant enrollment, scheduling, attendance, and program documentation.\nCoordinate three annual 12-week cohorts.\nPrepare program materials and participant packets.\nEnsure smooth daily operations and adherence to program policies.\nMaintain confidentiality and compliance with HIPAA and agency policies.\nParticipant Engagement\nBuild trusting, respectful relationships using trauma-informed and harm reduction principles.\nWelcome and orient new participants.\nProvide ongoing encouragement and motivational support.\nConduct regular participant wellness check-ins.\nAssist participants with referrals to healthcare, behavioral health, housing, treatment, and social services.\nCoordinate with Partner Agencies on referrals supporting the participants.\nPromote participant retention throughout the program.\nPartnership Coordination\nServe as the primary administrative liaison between The Gubbio Project, SFAF, and SFDPH.\nCoordinate scheduling with facilitators, clinicians, and outreach staff.\nAttend multidisciplinary team meetings.\nSupport communication between all program partners.\nIncentive Management\nManage Contingency Management incentive distribution.\nMaintain accurate inventory and secure storage of gift cards and participant incentives.\nTrack participant milestones and incentive eligibility.\nEnsure compliance with funding requirements and audit standards.\nData Collection & Reporting\nMaintain accurate participant records.\nTrack attendance, incentives, outcomes, and engagement.\nPrepare reports required by SFDPH and funding partners.\nAssist with program evaluation and quality improvement.\nMonitor program performance measures.\nGroup & Event Support\nAssist with Spanish-language group logistics.\nCoordinate participant Water, coffee, meals, and supplies.\nSupport educational workshops and wellness activities.\nEnsure meeting spaces are prepared and welcoming.\nMaintain an information table with updated printed Spanish information on services and resources.\nSet up and clean up of the group session space\nAdministrative Responsibilities\nPurchase program supplies and meals.\nManage program calendars.\nMaintain organized files and documentation.\nAssist with grant reporting and compliance activities.\nSupport budget tracking related to program expenses.\nCoordinate PROGRESA team meetings, take notes, and follow up on action items and agreements.\nQualifications\nRequired\nBachelor's degree in Social Work, Public Health, Human Services, or related field (or equivalent lived and professional experience).\nMinimum 2 years of experience in case management, harm reduction, behavioral health, community outreach, or program coordination.\nFluent in both English and Spanish (written and verbal).
