PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.\nABOUT THE JOB\nThe program manager is responsible for the program's day-to-day operations and supervision of service delivery staff. The incumbent will collaborate with both internal and external departments to enhance accessibility and service delivery for program participants.\n2 positions. Shift: Sunday-Thursday, 11:00am - 7:30pm or Tuesday - Saturday, 3:00pm - 11:30pm\nPosition Responsibilities:\nProvide leadership to the multidisciplinary teams, ensuring continuity of care and services between PATH agency resources and partnering agencies\nProvide individual and team supervision, support, and performance evaluations for assigned staff\nEnsure that services emphasize trauma informed care, client safety, and harm reduction interventions.\nBe available to respond to emergency calls and situations as required by the contract and organization\nAssist in the screening, assessment, and enrollment of clients in a range of services\nEnsure all paperwork and data collection is complete, timely, accurate, and current in agency records and electronic databases through ongoing audits of electronic databases and chart reviews\nEnsure adherence to funding source contracts and performance standards; prepare written reports on program activities as assigned and as required by funding sources\nWHAT YOU BRING\nWe’re looking for candidates with:\nAnalytical Skills and Attention to Detail: Gather and interpret data, identifying trends, patterns, and insights to inform decision-making.
