Job Description: LVN/LPN - Home Health Position Type: Contract Shift: Day Shift (5×8 Hours) Schedule: Typically Monday-Friday with possible rotating weekend assignments Patient Population: Adults & Geriatrics Position Overview We are seeking an experienced Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for a Home Health assignment. The nurse will provide skilled in-home patient care, perform clinical assessments, and support patients recovering from illness, surgery, or chronic conditions. Key Responsibilities Provide skilled nursing care during home health visits. Conduct comprehensive cardiopulmonary and physical assessments . dminister medications and injections as prescribed. Perform wound care and dressing changes , including compression dressings. Provide PICC line or midline care and dressing changes . Perform catheter insertion and care . Manage PleurX / Asept drain care when required. Conduct staple and suture removal . Monitor blood glucose using a glucometer and perform protime testing when needed. Educate patients and caregivers on disease management, medications, and home care procedures. Document patient care accurately using electronic health record systems. Coordinate with interdisciplinary teams including physical therapy, pharmacy, and social services . Patient Care Details Skilled home health visits typically 6-8 visits per day . Common patient diagnoses may include: Heart Failure COPD Pneumonia Stroke Post-operative recovery Other general medical conditions Qualifications ctive LVN/LPN license . Minimum 1 year of home health or related clinical experience . Must have recent direct patient care experience . Experience with cardiopulmonary assessment and wound care required. Strong clinical and documentation skills. bility to work independently in a home care environment. Additional Requirements Experience with electronic charting systems preferred. Must meet all required compliance, orientation, and competency standards before assignment start. Candidates should be flexible with scheduling based on patient care needs.
