Starting Point Senior Information Specialist/GUIDE/Caregiver Advisor Support

100% ON-SITE POSITION. NO HYBRID OR REMOTE ARRANGEMENT. Provides information, referral, and assistance to those that qualify under the Aging and Disability Resource Center. Implements and provides ongoing GUIDE programing with eligible Caregiver and CCP clients. Education and Experience Minimum of Bachelor’s Degree in health/human services or related field or RN with one year of experience with community agencies and a working knowledge of services available in Macon County including, but not limited to those serving seniors and people with disabilities. Must have excellent communication, organizational, and decision-making skills. Must have the ability to work with diverse community groups. Must have a dependable vehicle for use during work hours, valid Illinois Driver’s License, and appropriate auto insurance. Must be physically able to get in and out of a car many times per day in all types of weather and be able to adapt in a positive manner to various client living conditions (i.e. dirty, cramped). Must be able to carry, pull, and manage a minimum of 30 pounds of equipment daily. Duties and Responsibilities Senior Information Servies duties and responsibilities: 1) Provides information, referral, and outreach services by phone or face-to-face visits, in the office and/or in the home, with an older individual and/or her/his caregiver(s). 2) Trained on Options Counseling and provides Options Counseling. 3) Accesses appropriate GAP, ISC and ESS funding. 4) Maintains current information with respect to the services and opportunities available to seniors and/or people with disabilities. 5) Enters Information & Referral, Caregiver and ISC information and units into Aging Connections Official Reporting Network (ACORN). 6) Enters client contact forms and Public Media/Activity Forms in the ACL/STARS reporting system within appropriate timeframes. 7) Completes SIS POMP surveys monthly. 8) Obtains and maintains Senior Health Insurance Program certifications, INFORM USA and SAGE certifications. Submits proof per Division protocol. 9) Participates in SHIP calls. 10) Attends the Central Illinois Care Network monthly meetings. 11) Assists in conducting community outreach specific to Increasing Social Connectedness (ISC), including but not limited to, presentations on Starting Point Services. 12) Assists in provision of ISC services to older individuals, including home visits when necessary. GUIDE and Caregiver Advisory Services duties and responsibilities: 13) Completes GUIDE training and is integral in implementing the GUIDE program, beginning with current Caregiver and CCP participants. 14) Enters GUIDE information into Eli Health Guide platform in a timely manner. 15) Obtains and maintains Diabetes Self-Management Program (DSMP) and Stressbusting for Caregivers certifications and teaches required courses. 16) Completes CAS POMP Surveys monthly. 17) Assists with CAS/GRG support groups and programming when necessary. 18) Utilizes the CG/GRG log to assist with monthly contacts on Guide participants. Enters each contact into the log in a timely manner. 19) Participates in Caregiver Advisor quarterly calls held with ECIAAA. 20) Promotes, markets, and completes public relations activities for the Caregiver Advisory Services Program, Senior Information Services Program, and SHIP program through presentations, networking, and print/media promotions. General Duties: 21) Maintains case records, including documentation and follow up. 22) Refers, coordinates, and networks with other agencies in the community as well as within the Health Department to meet the needs of the caregivers. 23) Checks emails, voicemails, and inbox daily. 24) Attends required trainings which may include out of town travel. 25) Completes required paperwork and reports within required timeframes. 26) Performs other duties as requested by the Supervisor(s). The use of a particular expression or illustration describing the duties shall not be held to exclude other duties not mentioned that are of similar kind or level of difficulty. 27) Retains knowledge about all Macon County Health Department (MCHD) programs in order to provide appropriate referrals. 28) Acts as a mandated reporter of child and elder abuse and reports incidents to the appropriate agency(ies). 29) Adheres to MCHD personnel policies. 30) Complies with MCHD policy to be on call 24 hours per day 7 days per week in an emergent situation in Macon County or elsewhere as needed. 31) Signs Confidentiality Form and adheres to confidentiality policies. Responsible at all times to protect the confidentiality of each client’s file. 32) Required to pass Alcohol/Drug screen, Criminal Background Check, and Motor Vehicle Record Check. 33) Abides by the Health Insurance Portability and Accountability Act (HIPAA) policies and procedures as established by the Macon County Health Department. 34) Professionally represents the MCHD as evidenced by appearance, conduct, and respect to customers, co-workers, and employees of other community services, businesses, etc. 35) Promotes customer service, customer satisfaction, and customer retention by providing a professional, friendly, service-minded approach to all customers. 36) Promotes and enhances the functions and mission of the MCHD by supporting the agency and its employees in a positive manner and providing assistance when needed.
