Take this chance to make a difference in the lives of our patients and their families.We are currently recruiting for an RN or LPN to reliably serve patients in the following areas:Jasper + surrounding countiesFlexible scheduling -Part Time Visits [10-15 Monthly RAI Visits]No Nights, No Weekends, No Holidays, No On-CallMileage paidPay Per Visit [PPV]What This Role EntailsThis role plays a vital part in supporting Medicaid clients receiving in-home services by conducting annual Resident Assessment Instrument (RAI) and Global Health Evaluation (GHE) assessments. These assessments help identify each client's strengths, needs, and overall health status to ensure they receive the appropriate level of care and services.Through observation, assessment, and collaboration, you will gather and document critical information used by the State of Missouri to determine continued eligibility and reauthorize in-home services.
