Social Worker

Social Worker Landmark Hospital of Athens has an exciting opportunity for Social Worker to join our dedicated and professional team. Summary: The Social Worker will work in collaboration with members of the interdisciplinary team to enhance the cost-effective utilization of resources by identifying and initiating complex discharge planning to accomplish the timely transition of patients to post-hospital settings. Responsibilities: Provides assistance to patients, families, and others with a wide variety of needs to include, but not limited to, discharge planning, financial concerns, and emotional support as related to the current acute illness. Provides supportive counseling to patients regarding adjustments to illness as well as other biopsychosocial needs as indicated. Communicates with external agencies to provide referral information for a smooth transition from hospital or the post-hospital setting. The ideal candidate must be highly detailed and goal-oriented, with superior analytic, critical-thinking and time-management skills. They must have a positive and empathetic personality with the ability to work effectively with others as part of a multidisciplinary team in a complex care environment. It is also vital that they have excellent communication and interpersonal skills and consistently display a professional attitude. Requirements: Master’s degree in Social Work Active and unrestricted Georgia LMSW or LCSW Minimum of 2 years hospital experience Must have in depth knowledge of system and community resources and referral agencies Ability to communicate effectively with other healthcare agencies and practitioners Team, detail and goal oriented Positive, professional and empathetic demeanor About us: Landmark Hospitals is a privately owned hospital system with locations in Georgia, Missouri and Florida.
