This position ensures accurate and timely completion of the MDS process, care planning, documentation, and reimbursement coordination while supporting nursing operations and regulatory compliance.Essential Duties & ResponsibilitiesMDS & Care CoordinationCoordinate and oversee the completion of resident assessments and MDS schedulesEnsure assessments are completed accurately and within required timelinesDevelop, coordinate, and update individualized resident care plansManage Medicare and Medicaid assessment scheduling and coverage processesAssist with reimbursement documentation and denial notifications as neededMaintain current knowledge of Medicare and Medicaid regulations and updatesClinical & Administrative ResponsibilitiesSupport day-to-day nursing operations in accordance with facility policies and regulatory guidelinesCollaborate with interdisciplinary departments to ensure continuity of resident careParticipate in quality assurance and performance improvement initiativesAssist with discharge planning and resident transitionsParticipate in state and federal surveys and audits as requestedComplete administrative reports, medical forms, charting, and evaluations as requiredMaintain confidentiality of resident and facility information in compliance with HIPAA regulationsDocumentation & ChartingEnsure accurate and timely clinical documentation for admissions, transfers, and dischargesReview physician orders, treatment plans, and resident records for accuracy and complianceMaintain detailed nursing notes and incident reportsEnsure all charting meets facility, state, and federal standardsMedication & Clinical OversightAdminister medications and treatments as ordered, when necessaryMonitor medication records and narcotic documentation for accuracyReport discrepancies and ensure compliance with medication administration policiesTeam Leadership & Staff SupportAssist with staffing coordination and shift planningSupport nursing staff compliance with policies, procedures, and job responsibilitiesParticipate in employee evaluations and staff developmentCommunicate staffing concerns and operational needs to nursing leadershipQualificationsCurrent and active California Registered Nurse (RN) licensePrevious MDS Coordinator experience preferredStrong understanding of Medicare, Medicaid, OBRA, and skilled nursing regulationsExperience in long-term care or post-acute care preferredExcellent organizational, communication, and leadership skillsAbility to manage multiple priorities in a fast-paced environmentKnowledge of electronic medical records and clinical documentation systemsJoin Our TeamAt Moraga Post Acute, we are committed to providing compassionate, high-quality care while supporting the professional growth of our employees.
