More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are living in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today.\n\nStability Supportive Housing Program provides 60 units of scattered site supportive housing to chronically homeless single adults and adult families with a family member who has a serious mental illness and/or substance use disorder. The apartments are located throughout New York City.\n\n Position: Case Manager\n\nReports To: Assistant Program Manage\n\nLocation: Brooklyn NY 11225\n\nWhat Does a Case Manager Do:\n\nProvide all contract-funded activities, including conducting Intakes, Assessments, Service Plans, Reassessments, Service Plan Updates, Accompaniment, Referrals, Advocacy, Housing Inspections, Case Conferences, and other program requirements.\nComplete all program documentation and enter client services into Client Track, ensuring records are accurate and up to date.\nEnsure clients are connected to and maintain primary medical care, and adhere to medical and medication treatment plans.\nConnect clients to needed services, such as, medical, mental health, substance use treatment, as well as, legal, entitlements, etc.\nMonitor clients' progress toward their goals (dates achieved) via regularly scheduled telephone contact and/or face-to-face home and office visits, and document via progress notes.\nFunction as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc.\nEscort clients to appointments (educational, medical, social service, etc.)\nAssist clients in completing applications for benefits and entitlements and monitors outcome of process.\nCollect all required documentation and create client files.\nConduct a minimum one home visit per month\nConduct program required unit inspections as required by funding source.\nRequest Emergency Assistance to assist clients in paying clients’ portion of utility bills, if needed.\nInput work orders into the system and follow-up on repairs, which may include contacting building management and coordinating between outside contractors and clients\nMonitor clients’ living conditions.\nRespond immediately to emergencies and inform supervisor.\nMeet required monthly projections.\nProvide all required information for the completion of monthly program reports.\nMaintain clients’ charts always audit ready.\nProvide all required information for weekly/monthly/quarterly/annual reports to CAMBA management and/or to funders.\nParticipate in administrative and staff meetings as requested.\nOther duties as assigned.\n\nWhat are the Qualifications for the Position:\n\nBachelor’s degree with one year of experience, or\nAssociate degree with two years of experience, or\nHigh school diploma or GED with four years of experience\n\nMinimum Experiences and Skills Required:\n\nExperience as described above, preferably in a mid-sized (250+ employees) organization working with persons with a histories of homelessness, mental illness, and/or substance abuse.\nGood written & verbal communication.\nComputer literacy in Microsoft Office Suite.\n\nPreferred Qualifications:\n\nBi-lingual English and Spanish.\n\nCompensation: $54,672.80 annually\n\n Status: Full-time (35 hours weekly)\n\nCAMBA is an Equal Opportunity Employer.
