Grant Admin/Clinical Data Analyst/Community Health Worker

Job Summary: A versatile healthcare professional supporting Blue Ridge Medical Center through grant management, data analysis, and community outreach. Responsibilities include developing and managing grant proposals, securing funding from government, foundation, and corporate sources, and ensuring compliance with reporting deadlines. Conducts clinical data analysis, reporting, and validation to support care management, quality improvement, and performance initiatives, while coordinating cross-departmental data projects. Engages directly with community members to provide health education, basic screenings, social support, and advocacy, helping address population health needs. Combines strategic planning, analytical skills, and community engagement to advance organizational goals and improve health outcomes.May also be assigned to special projects and must demonstrate a sincere dedication and loyalty to the mission, vision and core values of BRMC.Grants Administrator 50%The Grants Administrator serves to achieve grant writing and management efforts. The most crucial responsibility is to develop grant proposals and manage active grant support from government agencies, foundations, and corporations to meet the financial and operational needs of BRMC. The Grants Administrator must: be highly aware of the internal workings of the organization to effectively identify appropriate funding opportunities; collaborate with administration to write proposals, letters of inquiry and grant proposals; communicate with granting agencies, track the status of grant requests and submit evaluation reports to funding agencies always assuring deadlines are met. The Grants Administrator must exercise discretion and independent judgement concerning matters of BRMC significance. May also be assigned to special projects and must demonstrate a sincere dedication and loyalty to the mission, vision and core values of BRMC.Essential Job Duties and Functions:Assist the Director in performing research to identify available grant opportunities and develop summaries of grant opportunities for management.Write grants in a timely manner, allowing for review and collaboration with Director and administration prior to submission.Work with finance department for budget development, provide budget reports and follow financial guidelines of funding sources.Monitor progress in meeting strategic objectives and key initiatives of grants. Assure critical deadlines are met.Provide technical and administrative assistance to staff and administration according to grant guidelines.Keep staff informed of upcoming deadlines and deliverables, thereby ensuring smooth completion of work responsibilities.Provide timely close out of expired projects through the coordination of financial and administrative departments.Serve on committees as appropriate to support the mission of BRMC.Maintain active accounts for all needed grant-related websites including the HRSA-EHB.Clinical Data Analyst (CDA) 30%The Clinical Data Analyst (CDA) maintains and examines clinical data for Blue Ridge Medical Center (BRMC). Typical duties include data reporting, data analytics, and data mining, as well as coordinating with relevant departments to determine areas for analysis and the appropriate metrics to ensure data analysis is optimally useful. Develop plans for data-related projects, identifying necessary resources and determining timelines and milestones. Other tasks performed may include performing data validation, handling data reconciliation, and working to retrieve missing data as needed. This position is principally focused on IT support of care management, clinical quality, and performance improvement.Essential Job Duties and Functions:Utilize LEAN, Six Sigma, Plan-Do-Study-Act and other performance improvement methodologies for process improvement.Utilize programs and databases, including Health Landscape, UDS mapper, Google Maps and other GIS for population management, healthcare trends, provider shortages, and opportunities for performance improvement, service expansion and grants.Collaborate with Information Technology and clinical staff to support defined business and clinical needs.Develop and analyze systems to obtain statistics necessary for improving quality of care and care management processes.Review, extract, compile and analyze patient data such as medical, financial, coding, and population health records.Recommend standard reports and data monitoring and trending solutions.Abstract data using predetermined criteria.Develop mechanisms to approve and distribute studies and reports.Review reports for validity and develop report archive.Identify data sources to prepare studies and reports and maintain data abstracting process.Support and facilitate interdepartmental performance improvement activities to enhance care integration.Community Health Worker (CHW) 10%The Community Health Worker will primarily work in the community with specific target populations, providing social support and informal counseling, advocating for individuals' and community health needs, and providing basic health screenings.Essential Job Duties and FunctionsParticipate with implementing program strategies to achieve organizational goals and objectivesCollaborate with staff and community partners to identify and address community needsEnsure program activities align with BMRC's mission and valuesMaintaining an elevated level of confidentiality and integrity.Develop and maintain relationships with BRMC leadership and clinical team, community partners, stakeholders, and fundersRepresent BRMC at community events and meetings as neededCollaborate with partners to identify opportunities for program growth and improvementCollect, analyze, and report on community health data to identify key health trends, gaps in care, and areas of need.
