LPN ADULT CARE - Day Shift

Performs and coordinates a variety of tasks to accomplish individual resident care. Responsible to respond to emergency calls for the Adult Care Facility. Observes and reports health and/or behavior changes to the RN and physicians as needed. Salary range: $26.00 - $30.00 per hour + $2.00 shift differential Responsibilities: Assists residents with their medications and performs prescribed treatments. Observes, reports and documents change in resident health and/or behavior. Coordinates communication and documentation of resident conditions with physicians. Offers suggestions establishing care plans for residents to maintain consistency and continuity of care. Assists in admission, transfer and discharge of all residents assuring proper orientation to the facility. Coordinates staff scheduling for coverage of Adult Care Facility if necessary. Assists in orientation of new employees. Assures that new employees understand scheduling process. Assist on floor to provide personal care to Residents as needed. Supervises staff routines in the Adult Care Facility. Counsels and/or issues counseling statements as indicated by the Director. Qualifications: At least one years’ experience working in the health care field. Must be currently licensed as a Licensed Practical Nurse in the State of New York.
