Our focus on lifestyle medicine and value-based care aligns with our mission to deliver compassionate, high-quality care!About The Role:Health by Design is seeing a full-time Nurse Navigator; the Nurse Navigator provides support to enhance the health of a unique patient population, specifically retired First Responders and their dependents. Will be responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating options and services required to meet an individual's health and wellness needs, using communication and available resources to promote quality, cost-effective outcomes. This individual will work between two (2) clinic locations, Schertz (NE) and Fair Oaks (NW), as well as in our administrative offices located in the Shavano Park area. After 90 days, there may be an opportunity for the role to become hybrid. Monday - Friday, 8:00am - 5:00pm. NO 12-hour shifts, NO weekends and NO major holidays.Wonderful work/life balance! Explore our beautiful clinics virtually: https://www.thefundsa.org/videogallery/#locationsResponsibilities:Facilitates patient wellness and autonomy through advocacy, communication, education, and identification of service resources.Develop a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a patient's overall wellness through integration to ensure continuity of care.Provides medication, diet, and other instructions as ordered by the provider and recognizes opportunities for individualized health counseling with patients and families/caregivers.Organize, prioritize, and coordinate the clinical data of patientsEstablish/maintain good rapport with clients, work closely together to achieve program goals.Conduct telephonic outreach to at-risk participants based on diagnosis and treatment plan.Arrange and lead health education events and programs, including lunch and learns and other speaking opportunities.Coordinate program marketing efforts to promote patient engagement.Help identify appropriate providers and facilities for the continuum of services while ensuring resources are being used in a timely and cost-effective manner to obtain optimal value for both the patient and the client.Establish/maintain good rapport with internal and external providers to obtain necessary information.Enter data into the appropriate system and prepare all written communication to the patient and/or provider.Achieve an in-depth knowledge of the client benefit plan.Process and maintain confidential information according to the confidentiality policy.Communicate, collaborate, and cooperate with internal and external stakeholdersComply with HIPAA Regulations.Ensure safe care to patients, staff, and visitors and adhere to all Health by Design policies, procedures, and standards within budgetary specifications.Graduate of an accredited school of nursing (ADN or BSN).3-5 years CLINICAL experience and 2-3 years' experience in care managementhealth coaching or equivalent experience.Current unrestricted Texas licensure to practice as a Registered Nurse, or bachelor'sdegree in social work required, CCM certification and/or Board-Certified Health andWellness Coach preferred.Knowledgeable and compliant with all relevant laws, rules regulations, andaccreditation standards and requirements.Strong clinical background in nursing or social services.Excellent verbal and written communication skills, including public speaking skills.Ability to perform multiple tasks simultaneously, work under pressure, and meetcritical deadlines.Build relationships to promote a collaborative environment.Focus on excellence and critical thinking skills.Excellent computer experience.Exceptional documentation skills.Ability to work independently, manage time, and prioritize projects.Demonstrates commitment to Health by Design by integrating our mission and valuesthrough innovation, accountability, empowerment, collaboration, compassion, andresults.Focus on service excellence and critical thinking skills.High EQ and relationship-building.Motivational interviewing.Attentive listening.Life/Health Coaching.Condition/age-specific plan of care.
