Your contributions will help shape our future and transform lives!\n\nJob Summary\nReporting to the Clinic Manager, the Care Manager RN coordinates the care and services of CIBD patients across the continuum of illness; promotes and monitors effective utilization of health care resources; and assumes a leadership role with the multi-disciplinary care team to achieve optimal clinical and resource outcomes. Operating in a Team Center Care model, the Care Manager adheres to the standards and regulations established by California Children’s Services (CCS), the Genetically Handicapped Persons Program (GHPP), the Joint Commission and other state and federally sponsored programs. The Care Manager RN is a vital member of the Case Management Team.\n\nDepartment: Clinic\nSchedule: M - F, 8:30 - 5:00\nType: On-site\n\nEssential Duties\nPatient Coordination: Collaborate with patients to identify their needs and understanding of the care plan. Coordinate their care with external facilities, including home health agencies, third-party payers, pharmacies, and lab facilities.\nEffective Collaboration: Work closely with internal partners (e.g., physicians, nurses, authorization coordinators) and external providers (e.g., home health agencies, pharmacies, lab facilities) to ensure seamless coordination of care and fulfillment of orders from initiation to completion.\nSelf-Management and Communication: Exhibit self-direction while effectively working with others. Demonstrate strong organizational and documentation skills. Employ effective written and verbal communication, utilizing strong computer skills for navigating the EMR, submitting electronic prescriptions, sending electronic approval requests, and engaging through email, Zoom, and Microsoft Teams. Use critical thinking and problem-solving capabilities to address conflicts and resolve issues while providing patient education.\nTeam Support: Promote a comprehensive care team model and be prepared to become cross-trained as necessary for staffing support. Maintain strong communication to foster positive interpersonal relationships within the team.\nProgram Development: Enhance the established program as it relates to the Care Manager’s assigned patient population through community outreach, engagement with school and community-based organization liaisons, active participation in committees, conference attendance, and additional duties.\nRequired Education/Experience\nGraduate from an accredited Registered Nurse (RN) Program.\nMinimum of two (2) years nursing experience in acute care or ambulatory setting.\nAbility to work onsite (not a remote role).\nRequired Licenses/Certifications\nActive California State Registered Nurse (RN) License.\nBasic Life Support certification (or obtain within first three months of hire).\nHIPPA training (or obtain within first three months of hire).\nAdditional Required Qualifications\nKnowledge of standardized nursing procedures in compliance with organizational policies and procedures.\nSolid understanding of regulatory requirements as set forth within nursing and clinic licensure.\nProficiency with Electronic Medical Record databases.\nPreferred Education/Experience/Skills\nBachelor of Science in Nursing (BSN).\nMinimum one year of direct, full-time Case Management experience.\nKnowledge of CCS/GHPP/Medi-Cal regulations and requirements.\nKnowledge of third-party payer methods and regulations.\nExperience in hematology nursing.\nExperience with adults and pediatric patients.\nBilingual English/Spanish.\nBenefits\n100% employee coverage and 50% dependent coverage of medical and vision; 99% employee coverage of dental\nLife insurance\nPaid Time Off\n10 observed paid holidays\n403b retirement plan with 4% match\nCompensation listed is the expected salary base at time of hire.
