accreditation team - and nursing staff to facilitate communication and ensure compliance with the Commission on Accreditation of Rehabilitation Facilities (CARF) standards for providing care coordination services to acute rehab patientsCoordinates the hospital's performance plan for the Stroke Program and Acute Rehab ProgramActs as a resource person and consultant for the education of staff and serves as the primary chair-member of the medical center stroke subcommitteeSupports activities at the unit level - trends and tracks important processes and activities in patient care and organizational functions for stroke and rehab patientsConducts care coordination functions such as patient education - as well as compliance with regulatory bodies and national guidelines for stroke managementFollows acute stroke and rehab patients to formulate a plan of care based on nursing assessmentsDelegates elements of care to the appropriate individual based upon assessed needs -
