The Post Acute Navigator receivest client information from multiple referral services (i.e. Hospitals, Physician offices, Inpatient Rehab, Outpatient Wound Care Centers, Skilled Nursing Facilities, etc). Responsible for the timely completion and tracking of referrals. Document referral information in Electronic Medical Recors and communicate to referral source.Core Responsibilities and Essential Functions:Referral ScreeningReview ordered services and Face to Face encounter for appropriateness and priority.Gather and review presence of qualifiying criteria for services orderedReview payor considerations related to provider selection to include MVP searchReview geographical location of services to be provided.Review for existing or patients preferred providerCoordination of ReferralCompile clinical information using PixCert and Electronic Medial RecordPerform double check of complete referralDirect referral to designated referral source within WellStar Post Acute Care or alternate provider as appropriateCommunicates with providers, facilities via fax/telephone/EMR to determine acceptances and arrangements of servicesContinue communication with difficult placement as appropriateCommunicate with Care Coordination and Wellstar Homecare to expedite dischargeComplete electronic documentation in medical record regarding coordination of services, complete other documentation and data entry as appropriate.Customer ServiceMaintain seamless transition for patients from inpatient acute and MD offices to outpatient post acute servicesMaintain positive professional liaison relationship with members of the inpatient acute, MD office and outpatient post acute teamsAttend and participate in discharge planning meetings to expedite timely and appropriate dischargesPerforms other duties as assignedComplies with all Wellstar Health System policies, standards of work, and code of conduct.Required Minimum Education: High School Diploma General or GED GeneralRequired Minimum License(s) and Certification(s):All certifications are required upon hire unless otherwise stated.Additional License(s) and Certification(s):Required Minimum Experience:Minimum 3 years experience in health care or preferably Post Acute Service Area, i.e., Homecare, Hospice,IRU, Nursing Home, LTAC, or Physicians office. RequiredRequired Minimum Skills:Professional, independent and self directed.Strong organizational and communication skills.Ability to multitask in a fast paced environment.Excellent communicate skills, both verbal and written.Possess knowledge of community resources and managed care contracts for post acute care.Computer proficiency a must.
