Job Details: Schedule: 8-4:30 pmLocation: Fort Smith, AZJob Type: ContractContract Length: 3 months (possibility of extension)Pay Range: $50/hr- $56/hr*Pay offered is based on experience, expertise, credentialing, and education. Coverage Areas: Fort Smith, AR and surrounding area Key ResponsibilitiesPerform comprehensive admission assessments and OASIS evaluations for home health patients. Develop individualized treatment plans based on patient needs, physician orders, and clinical findings. Provide skilled physical therapy interventions focused on improving strength, balance, mobility, endurance, pain management, and functional independence. Treat post-operative orthopedic patients and other individuals recovering from injury, surgery, or illness. Complete routine treatment visits and monitor patient progress toward established goals. Educate patients, caregivers, and family members on therapeutic exercises, mobility techniques, safety precautions, and home exercise programs. Accurately document patient care, evaluations, and visit notes using HomeCare HomeBase (HCHB). Collaborate with physicians, nurses, occupational therapists, and interdisciplinary healthcare teams to coordinate patient care. Ensure compliance with state regulations, agency policies, and home health documentation standards. Participate in weekend coverage rotation as required. Required QualificationsCurrent and active Physical Therapist license in the state of Arizona. Minimum of 1 year of home health physical therapy experience. Hands-on experience using HomeCare HomeBase (HCHB). Experience treating post-operative orthopedic patients. Strong knowledge of OASIS documentation and home health regulations. Current CPR/BLS certification. Valid driver’s license, reliable transportation, and current auto insurance.
