Care Manager - MSW or RN

We are looking for a licensed MSW or RN Care Manager toservice both seniors and adults with physical disabilities, mental health issues, dementia, andtraumatic brain injuries.Essential Job Responsibilities:• Assessment of new patients and maintain ongoing patient evaluations.• Create individualized care plans based on specific patient needs.• Respond and handle patient crises in a professional and timely manner.• Develop coherent, legible, and grammatically correct assessments, care plans, andmonthly reports on a timely basis.• Document all patient interactions in our online software program.• Represent the organization through positive interactions in the community.Compensation:$45 per hour$1000 hire-on bonus after first 90 days!Schedule:32 hours per week guaranteed with ability to increase!Hours are congruent with client needs.
