Location: Alameda, CASchedule: Monday – Friday, 8:00 a.m. – 5:00 p.m.Salary Range: $75K to $84K a yearPosition SummaryWe're hiring an Associate Mental Health Therapist to join our Alameda-based IBH program. This role is ideal for registered associates seeking structured supervision, hands-on experience, and meaningful client interaction in a team that values collaborative care across systems.Key ResponsibilitiesCare Coordination & Program SupportDeliver in-person and telehealth-based care coordination for individuals with behavioral health and social support needsConduct comprehensive client assessments and contribute to individualized care planningFacilitate warm handoffs and referrals to behavioral health, medical, housing, and community-based resourcesMonitor engagement and support follow-up care to maintain service continuityCollaborate with internal care teams and external partners to address client needs holisticallyParticipate in multidisciplinary team meetings, case reviews, and service planning sessionsDocument services and progress accurately in the electronic health record (EHR)Assist with data collection and service tracking to meet reporting requirementsCompliance & TrainingEnsure compliance with Medi-Cal, HIPAA, and internal documentation protocolsAttend required trainings, community forums, and IBH-related meetingsSupport program evaluation activities by monitoring outcomes and service deliveryContribute to outreach, resource navigation, and sustainability effortsRequirementsRegistered Associate Clinical Social Worker (ASW/ACSW) in the State of CaliforniaBachelor's or Master's degree in Social Work, Psychology, Public Health, or a related fieldMinimum 1 year of relevant experience in behavioral health, case management, or social servicesFamiliarity with Alameda County systems of care, including Medi-Cal and community-based resourcesUnderstanding of trauma-informed and culturally responsive service deliveryStrong communication, time management, and interpersonal skillsAbility to work collaboratively with both clinical and non-clinical team membersPreferred:Experience in integrated or primary care settingsProficiency in an electronic health record system (SmartCare or similar)Bilingual skills (Spanish or other languages) highly valuedReliable transportation and valid CA driver's licenseWork EnvironmentHybrid community-based and clinic-based settingOccasional travel within Oakland CAUse of standard office and telehealth equipmentMay require occasional evening or weekend flexibilitySupervision & SupportClinical supervision available to support licensure hoursOngoing training and mentorship provided through internal and external partnersOpportunity to grow within a supportive, multidisciplinary team environmentEqual Opportunity StatementThis position complies with all federal, state, and local regulations including HIPAA, CMIA, and Medi-Cal standards.
