If you are a natural leader and are work positively with others, this is a position for you.\nCreative Minds ILS/SLS services adults with developmental disabilities. Case Managers will support with the coordination of caregivers, and will work alongside other Case Managers, Program Supervisor, and Program Director. Case Managers will coordinate, and schedule client support and services, mentor and manage a team of direct care professionals, provide quality assurance, complete monthly, quarterly, and annual reports, and intake assessments.\nUnder the direction of the ILS / SLS Program Supervisor and Program Director; core duties and responsibilities include the following. Other duties may be assigned.\nScreens clients for intake appointment and/or referrals either in person, on the phone, or on emergency basis.\nProvide case management services, using both group and individual ILS/SLS model.\nCoordinate and arrange services hours, implement schedules for staff and recipient coverage. Ensure service hours are implemented within the authorized level of service.\nProvide crisis intervention services to manage immediate concerns/risk; provide initial support/assistance; and help develop and enhance the working relationship between the client and the Center.\nParticipate as a member of a multi-disciplinary team. To attend all scheduled staff meetings to assist in assessing the client’s needs, strengths, resources, and provide feedback regarding treatment planning.\nNetwork effectively and provide co-location orientations with DPSS, GAIN, Job Club, and other community resources as needed.\nComplete all chart documentation, reports, coordinated services plans, need assessments, outcome and measurements, and other received paperwork within the time frame required.\nParticipate in home visits to clients when necessary and safe to do so.\nProvide unmatched client customer service, direct service to service recipients when required, in accordance with the recipient’s service plan through the agency\nOn-Call availability in accordance with current policy in place\nGather relevant case-specific information, data and documents for client files, at-home binders, and updated reports to send to CSC.\nCoordinate and track cases with Program Manager, and provide support when needed\nCoordinate with other professionals such as trust attorneys and other vendors that may be relevant for the client\nEducation and/or experience requirement:\nHigh school diploma or general education degree (GED) with five years of related experience and/or training; or Associates degree with three years of related experience and/or training, or Bachelors degree with one year of related experience and/or training.\nPreferred candidate characteristics\nKnowledge/experience with the Regional Center and DDS protocol\nField experience\nFingerprint/background clearance required - no felonies or criminal record\nAcceptable DMV record\nValid CA Driver's License\nWorking and reliable vehicle, auto insurance\nHealth screenings clearance\nEligible to work in U.S.\nPay and benefits\nRate of pay can range from $24.00 to $26.00 per hour based on experience and education, may increase within pay range. Workdays are Monday through Friday, 8 hours a day.
