Weekend Admissions Nurse (LVN/RN) – Skilled Nursing Facility

Job Summary We are seeking an experienced Admissions Nurse (LVN or RN) to join our team on a weekends-only basis. This role is responsible for coordinating and completing all admissions to ensure a smooth, efficient, and compliant intake process for new residents. Candidates must have prior skilled nursing facility (SNF) admissions experience and be comfortable working independently in a fast-paced environment. Duties Coordinate and complete all new admissions and re-admissions Review hospital records, discharge paperwork, and physician orders Perform initial nursing assessments upon admission Ensure accurate and timely documentation in EMR Communicate with hospital discharge planners and families Verify orders, medications, and treatment plans Collaborate with interdisciplinary team to ensure seamless transition of care Maintain compliance with state and federal regulations Qualifications Active LVN or RN license (California) REQUIRED: Prior SNF admissions experience Strong knowledge of Medicare, Medi-Cal, and admission processes Experience with EMR systems (PointClickCare preferred) Excellent communication and organizational skills Ability to work independently and prioritize tasks Strong skills in patient assessment, vital signs monitoring, and diagnostic evaluation required. Ability to work independently during weekend shifts while maintaining excellent communication with team members.
