This role is responsible for coordinating and completing all admissions to ensure a smooth, efficient, and compliant intake process for new residents.\nCandidates must have prior skilled nursing facility (SNF) admissions experience and be comfortable working independently in a fast-paced environment.\nDuties\nCoordinate and complete all new admissions and re-admissions\nReview hospital records, discharge paperwork, and physician orders\nPerform initial nursing assessments upon admission\nEnsure accurate and timely documentation in EMR\nCommunicate with hospital discharge planners and families\nVerify orders, medications, and treatment plans\nCollaborate with interdisciplinary team to ensure seamless transition of care\nMaintain compliance with state and federal regulations\nQualifications\nActive LVN or RN license (California)\nREQUIRED: Prior SNF admissions experience\nStrong knowledge of Medicare, Medi-Cal, and admission processes\nExperience with EMR systems (PointClickCare preferred)\nExcellent communication and organizational skills\nAbility to work independently and prioritize tasks\nStrong skills in patient assessment, vital signs monitoring, and diagnostic evaluation required.\nAbility to work independently during weekend shifts while maintaining excellent communication with team members.
