You will be providing specific assistance with medication management, housing, counseling, support and advocacy. Additionally, responsible for engaging, educating, and offering support to individuals and their family in order to successfully connect them to culturally relevant health services including prevention, diagnosis, timely treatment, recovery management, and follow-up.\n\nKey Responsibilities\n\nClient Care\n\nProvides emotional support to alleviate fears of and barriers to accessing quality health care.\nAssists assigned clients to identify and progress toward achievement of their chosen goals.\nMotivate and educate individuals/families/caregivers about the importance of preventative services, medical conditions and recovery strategies.\nAdvocates for clients in all areas of treatment and helps them apply for and receive services.\nEmpowers clients in meeting their goals.\nProvides face-to-face contact with each client assigned.\nAdvocates for and assists clients and their families in accessing and receiving mental health treatment, medical care, housing, food and clothing as indicated.\nDevelops individual service plans for each client with their input whenever possible.\nMeets regularly with clients at Aspiranet sites, jobsites, homes, board and care facilities, skilled nursing facilities, psychiatric facilities and other appropriate sites.\nArranges for or transports clients to and from medical appointments.\nProvides prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation.\nCompletes paperwork as assigned in a timely manner.\n\nQualifications\n\nMinimum Requirements:\n\nBachelor's Degree in a social service/mental health related field and two years’ experience in that field\nKnowledge of psycho-social rehabilitation’s treatment and programming.\nAbility to work and communicate with staff, clients, families, community agencies and professionals.\nAbility to perform crisis intervention strategies.\nAbility to work effectively under stress and conflict.\nAbility to exercise appropriate judgment and decision-making.\nAbility to be flexible and adaptable in any given situation.\nAbility to work as a member of a team.\nAbility to work under time deadlines and pressures.\nAbility to work with minimal supervision.\nMeets all state-required employment conditions, including:\nDOJ fingerprint and Child Abuse Index clearance\nTB/Health physical\nValid CA Driver’s License, acceptable driving record, and proof of insurance\n\nPreferred\n\nBi-lingual in County Threshold language(s).\n“Lived” experience is a plus\n\nWork Environment & Schedule\n\nField Based position that requires traveling to client's home or community setting\nSchedule is Monday-Friday Day Shift, with flexibility to work some evenings\n\nWhy Aspiranet?\n\nCompetitive compensation, comprehensive benefits package, and a 403(b) retirement plan with employer matching.\nAccess to the free Aspiranet Employee Perks Program.
