Intake Specialist (Intake, Screening & Triage)

Intake, Screening & Triage Specialist Salary: $39,048.46 yearly ($18.77 per hour) plus excellent benefits. EOE Immediate full-time opportunity for experienced professional to assist older adults and adults with disabilities to access long-term care services. Candidate will be responsible for providing program education, client intake/screening, and for facilitating access to long-term care programs. Candidate will assist in the preliminary determination of programs and services best suited to serve the needs of the elderly and adults with disabilities. Excellent customer service skills required. Candidates must possess excellent oral and written communication skills, strong attention to detail, good organizational skills, and knowledge of service delivery system. Computer skills required. Bachelor’s degree in Social Work or related human services field. Experience in client screening, assessments, information & referral, case management, crisis intervention, or related work experience preferred.
