Every day, our teams serve hundreds of individuals across multiple levels of care - many arriving in crisis and leaving with stability, skills, and hope.As a Case Manager / Group Facilitator, you will be part of that impact - helping individuals move from crisis to stability and from isolation to connection.This role combines group facilitation and case management, allowing you to build strong connections with clients while supporting their overall treatment journey.You will plan and lead groups, support individual client needs, and collaborate with a multidisciplinary team to ensure clients are progressing and receiving the appropriate level of care.This position is ideal for someone who is engaging, organized, and passionate about helping others grow in a structured, supportive environment.Schedule: Monday - Friday | 8:30 AM - 5:00 PMESSENTIAL FUNCTIONS:1. Group Facilitation & Client EngagementPlan, organize, and facilitate psychoeducational and process groups using evidence-based practices and program curriculumCreate a safe, inclusive, and supportive group environment that fosters participation and growthManage group dynamics and navigate challenges with professionalism and confidenceProvide individual check-ins, support, and crisis intervention as needed2. Case Management & Care CoordinationSupport clients with resource coordination, goal setting, and treatment engagementAssist with discharge planning and continuity of careAct as a liaison between clients, clinical staff, and external providersHelp remove barriers to care and support overall client progress 3. Treatment CollaborationCollaborate with clinical staff to align group content and case management with treatment plansParticipate in multidisciplinary team meetings and case discussionsContribute to treatment planning and client progress reviews 4. Documentation & ComplianceMaintain accurate and timely documentation including group notes, attendance, and case management updatesEnsure compliance with confidentiality standards, ethical guidelines, and regulatory requirements 5. Program Development & GrowthSupport development and improvement of group curriculum and programmingIncorporate community resources and creative approaches into servicesParticipate in ongoing training, supervision, and professional development ADDITIONAL FUNCTIONS:Performs other duties as may be assigned.Follows and supports TPEG policies and procedures.Works collaboratively and cooperatively with internal and external partners.Represents TPEG, Inc., by embodying the mission, vision and values of the organization.Travels occasionally during the workday and on occasional overnight stay.
