Job Overview We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide essential nursing care to patients in various settings, including hospice, geriatrics, and senior care. As an LPN, you will play a vital role in patient assessment, administering medications, and supporting individuals with developmental disabilities. This position requires adherence to HIPAA regulations and a commitment to delivering high-quality patient care. Duties Conduct patient assessments and monitor vital signs to ensure optimal health outcomes. Administer medications and treatments as prescribed by physicians. Provide personal care assistance to patients, including bathing, dressing, and mobility support. Collaborate with healthcare teams to develop and implement individualized care plans. Educate patients and their families about health management and disease prevention. Maintain accurate patient records in compliance with HIPAA regulations. Assist with the management of Durable Medical Equipment (DME) as needed. Support patients undergoing dialysis or other specialized treatments. Participate in ongoing training and professional development opportunities. Skills Strong understanding of HIPAA regulations and patient confidentiality practices. Experience in hospice care or senior care settings is highly desirable. Knowledge of working with individuals with developmental disabilities is a plus. Proficiency in patient assessment techniques and documentation practices (ICD-10 familiarity preferred). Ability to work effectively in diverse clinical environments including operating rooms (OR). Excellent communication skills for interacting with patients, families, and healthcare professionals. Compassionate approach to patient care with a focus on empathy and support. Strong organizational skills to manage multiple tasks efficiently.
