Position Summary
The Home Health Physical Therapist (PT) is responsible for evaluating patient needs, developing individualized care plans, and delivering skilled therapy services to improve mobility, function, and overall well-being. This position works closely with interdisciplinary teams to ensure optimal patient outcomes.
Key Responsibilities
Provide direct patient care, including evaluation of functional needs and development of individualized treatment plans
Assess patient progress and adjust care plans to ensure optimal outcomes
Collaborate with physicians, nurses, and other healthcare professionals to coordinate care
Implement therapy interventions in accordance with established standards of care
Supervise and direct support staff as appropriate
Maintain accurate and timely documentation of evaluations, treatment plans, and progress notes
Educate patients and caregivers on treatment plans and home exercise programs
Promote quality care and contribute to continuous improvement initiatives
Qualifications
Graduate of an American Physical Therapy Association (APTA) accredited program
Current, unrestricted Physical Therapist license in the state of practice
Minimum of one (1) year of experience in a healthcare setting (home health preferred)
Knowledge and experience with OASIS
Strong clinical, communication, and organizational skills
Ability to work independently and manage a flexible schedule
Requirements
Ability and willingness to travel within the assigned territory (approximately 50%)
Valid driver’s license, reliable transportation, and current auto insurance
Ability to meet physical demands of the role, including lifting up to 50–100 lbs and assisting with patient mobility
Willingness to work occasional irregular hours or on-call schedules, if required

Pay: $85,000.00 - $105,000.00 per year
