Faith Community Nurse Coordinator

With the support of more than 30,000 team members, we promote a forward-thinking philosophy that’s built on a foundation of trust, dignity, respect, responsibility and clinical excellence.Responsibilities:The Faith Community Nursing (FCN) Coordinator supports the development, implementation, and evaluation of faith community nursing programs in assigned communities. Serving as a liaison between the healthcare system, local faith communities, and the community at large. The coordinator guides both paid and volunteer FCNs in providing holistic care that integrates physical, emotional, and spiritual well-being. Key responsibilities include recruiting and orienting FCNs, assessing health needs, coordinating and performing wellness screenings and education events, as well as presentations to nurses and clergy. The coordinator also maintains program data, ensures compliance of data collection from FCN Volunteer and self reporting on Community Benefit Form. Fosters partnerships with congregations and community organizations to align services with BayCare’s health priorities and Community Health Needs Assessments (CHNAs). The faith community nurse coordinator is responsible to coordinate care of the clients enrolled in the FCN Transitional Care Program. Responsible for transportation between facilities and to community events. Other duties as assigned.RequiredMinimum Qualifications:Education: Associate's degree in Nursing or Diploma Nursing.PreferredEducation: BSN (Bachelor of Science in Nursing).RequiredCertification: BLS (Basic Life Support) and RN (Registered Nurse).RequiredExperience: Minimum 3 years of direct patient care experience.
