The PC will also be responsible for overseeing the implementation of each person’s served Coordinated Service and Support Plan (plan of care). The PC will ensure that they and their staff are providing the necessary support to each individual to ensure that their needs, interests, and preferences are respected and provided for in a safe and healthy environment that maximizes personal success.
