Outpatient Case Manager - Contract
Job Description:
In this role, you will facilitate activities that empower individuals to meet their goals. This will involve coordinating services with various community partners including healthcare providers, legal systems, housing, social services agencies, and relevant education programs. You will provide comprehensive, whole-person care, tailored to each individual's unique needs and cultural background, and help them reach their goals with tact, tolerance, and a steady mature presence. Additionally, you will support individuals in crisis situations and provide follow-up care through a crisis rotation.

Required:
A valid driver's license
Ability to pass a criminal background check
Bachelor's degree in social work, psychology, behavioral health, or a related field or a combination of at least 3 years of work and school experience
Certified Qualified Mental Health Associate (QMHA) or willingness to obtain your QMHA certification in Oregon
Preferred:
Experience with and/or understanding of individuals and families with behavioral health needs
Knowledge, Skills, and Abilities:
Ability to communicate effectively in both written and oral formats with clarity and purpose, including the ability to complete objective, accurate documentation in a timely, consistent manner
Ability to maintain confidentiality, comprehend laws, regulations, and policies, and the ability to follow such laws, regulations, and policies and report when they are being violated
Ability to interact with individuals of various backgrounds, sexual and gender orientations, education levels, and socio-economic status effectively, tactfully, and empathetically with a culturally responsive nature, especially with those experiencing a crisis and in stressful situations
Ability to provide care coordination and work collaboratively with community partners to ensure safer and more effective care for the individual
Must have strong computer skills including the ability to document in an electronic medical record, use electronic time tracking, and the ability to use Google Workspace, specifically Drive, Meets, Spreadsheets, Word documents, and email

45-49$/hr
Shared Housing Provided
Vehicle Provided up to 225 Miles Per Month OR reimbursement assistance to transport vehicle on-site
