POSITION SUMMARY: The Housing Case Manager provides case management services to homeless individuals diagnosed with HIV/AIDS in the community, assisting participants in securing permanent housing, and provides referrals for supportive services to community partners, all with the intent of maximizing self-sufficiency and moving people into housing as quickly as possible.

The goal of the SSVF Housing- Equity and Inclusion Case Manager is to assist veterans in ending their housing crisis, POSITION TITLE: Housing Equity & Inclusion Case Manager.

Case Manager, Case Management, Housing Specialist, Housing Coordinator. Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.

The Case Manager is the Person with primary responsibility for working with Veterans Place residents to set and track Personal goals, sustain a healthy lifestyle, and progress toward independence and permanent Housing.

The Family Services PSH Case Manager is responsible for the overall delivery of services and implementation of individualized case plans for the families in the Permanent Supportive Housing (PSH) program, including crisis intervention counseling, harm reduction, housing stabilization, job search, and maintaining permanent housing.

Familiarity with best practices in homeless services, such as Housing First, Harm Reduction, Motivational Interviewing, Strengths-Based Case Management, and Trauma-Informed Care is preferred.

Reporting to the Clinical Coordinator, the Case Manager, Transitional Housing will provide case management services to people experiencing homelessness, some with mental illness and/or substance use disorders, in order to assist the individual in the transition to permanent housing.

The Rapid Re-Housing (RRH) Case Manager is responsible for building relationship and individual case management for youth experiencing homelessness and highly vulnerable youth including survivors of human trafficking, within the CHA Rapid Re-Housing Program.

The Program Manager oversees Project 180’s ICMS Project Based Housing program, an innovative program that provides on site wrap-around behavioral health treatment to approximately 200 individuals.

The Case Manager collaborates with Clients to learn independent living skills and other life-skills vital to successful transition from homelessness into permanent housing. Through the use of evidence-based practices such as harm reduction, housing first, and trauma-informed care, the Case Manager supports Clients' housing stability, self-sufficiency, and community integration.

The goal of the SSVF Case Manager is to assist veterans in ending their housing crisis, enhance their independent living skills by providing supportive services and education, connect them with community resources, and empower them to maintain long-term housing stability and self-sufficiency.

The DV Housing Case Manager provides advocacy-based counseling and case management services including assessment, information and referral and goal setting and is responsible for cooperative case planning and coordination of services for residents.

Reporting to a Case Management Supervisor, the Case Manager will be primarily responsible for conducting psychosocial assessments with newly enrolled clients, co-developing treatment plans, assisting with obtaining and retrieving vital documents, assist with enrollment in benefits and entitlements and preparing or gathering clients documents needed for housing placement.
