Overview: CASES seeks a Clinical Case Coordinator to join its Intensive Services Unit within Pretrial Services. The Clinical Case Coordinator will be an active partner in providing direct services to participants primarily in the community with additional services being provided at other CASES program locations.

The Case Manager will work on assignment through our partnership with the Home & Community Service (H&CS) program of the Chester County Intermediate Unit (CCIU). Insurance and case management / case specialist related experience.

The Therapy Coordinator (TC) is responsible for supervising the rehabilitation department to meet resident needs, the company’s fiscal requirements, and federal and state regulations. The Therapy Coordinator will achieve specific patient care, customer service, and financial objectives through effective scheduling of staff according to patient care needs.

Two (2) years of experience as a Physical Therapist, Physical Therapist Assistant, Occupational Therapist, Occupational Therapy Assistant, Speech-Language Pathologist Therapist, or Rehab Tech. Must hold a current and active license as one of the following: Physical Therapist, Physical Therapist Assistant, Occupational Therapist, Occupational Therapy Assistant, or Speech-Language Pathologist; if the person is a therapist.

NCI Supportive Services for Veteran Families (SSVF) program aims to provide critical time intervention, strength-based case management approach to Veterans and their families with a desire to take action to break the cycle of crisis and vulnerability in their lives and change the trajectory of their lives through sustainable permanent housing interventions and foster a deeper understanding of the life-changing work carried out by The Salvation Army North and Central Division.

Reporting to the Shelter Director, the Program Coordinator (Overnight) will support our organizational vision towards ensuring a safe and supportive environment for all clients, staff and visitors while providing our clients with the best possible care and services.

Reporting to the Client Services Supervisor, the Case Management Pilot Program (CMPP) Coordinator leads the implementation of the CMPP in Baltimore. The CMPP Coordinator will supervise a team of CMPP Case Managers.

The Case Management Training Program is a Ryan White Part A funded program created to provide training to providers funded to deliver Medical Case Management (MCM) and non-Medical Case Management services to People Living with HIV (PLWH) within the Ryan White Boston Eligible Metropolitan Area. The PCIII will partner organizations to provide training to case managers, to ensure PLWH can reach and maintain viral suppression by accessing the full continuum of HIV services.

The Case Manager/Investigator will report to the Associate Director of Threat Assessment and Management and works integrally with partner units, including Student Affairs, Human Resources/Equal Opportunity Compliance and University Police.

Case Managers must meet unit performance measures and targets each month, ensuring individuals on their caseload receive the needed services and supports. Case Managers at PermiaCare maintain an assigned caseload of individuals and are responsible for providing required services to each individual, including keeping accurate records and documentation as required and within required timeframes.

The Case Manager (MSW) is responsible for coordinating and overseeing the delivery of healthcare services to elderly participants, conducting assessments, developing care plans, collaborating with the interdisciplinary care team, and advocating for the participants' needs.

Certified Case Manager (CCM), Certified Alcohol and Drug Counselor (CADC), or Mental Health First Aid. As a Senior Level Case Manager at a temporary homeless shelter, housing and (ECM) Enhanced Case Management, your role will involve a more specialized higher level of experience in comprehensive support services to individuals and families experiencing homelessness.

Works collaboratively with the Case Manager, Brewster Ambulance Services Transportation Coordinator and the VPNE care van ambassador to coordinate the various modes of discharge transportation.
