Job Summary: The Community Health Worker/Eligibility Specialist (CHW) is a frontline health worker who is a trusted partner with the community they serve. The Community Health Worker will have access to PHI in the course of their duties.

The Community Health Worker II (CHW-II) is responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors.

The NorthStar Community Health Worker (CHW) will be the primary care navigator for members with complex behavioral health needs. The CHW works assertively to locate and engage members in the community to develop individualized plans of care that focus on the member’s definition of success and identify and address social determinants of health (SDOH) or other barriers impacting members ability to achieve their stated goals.

3+ years of experience working within the Community Health setting in a field-based healthcare role. Engage with members post-discharge from inpatient setting to facilitate connections with community-based services and Peer Support Specialist as needed.

Prior experience as a Community Health Worker, Peer Support Specialist, or similar role a plus. We are currently recruiting for an experienced, personable Community Health Worker to work in patient s homes and virtually to deliver chronic care management to high complexity patients.

Three years of professional work experience as Community Health Worker, Patient Care Navigator, or related role supporting community members in navigating resources and promoting health.

SUMMARY The Community Health Worker (CHW) serves as an integral member of the CICOA team including a Supervisor, Behavioral Health Clinician, Care Managers (CM), and Care Coordinators (CC) working together to provide additional services and supports to individuals with a serious mental illness (SMI) diagnosis that CICOA serves.

Partner and collaborate with the ICT, community-based organizations, and inpatient/outpatient providers to ensure members can access medical, behavioral, and substance use treatment services to meet their healthcare needs.

The Community Health Worker must demonstrate a commitment to providing health care to families in medically underserved communities, must demonstrate outstanding customer service and the key behaviors , outlined in the Yale -New Haven Hospital core success factors and standards of professional behavior.
