The Targeted Case Manager will provide Targeted Case Management, assessment, and treatment planning services to patients with a substance use disorder. Targeted Case Management services at BrightView consist of: (1) assessment of the eligible patient to determine service needs; (2) development of an individualized integrated action plan; (3) referral and related activities to help the patient obtain needed services; (4) monitoring and follow-up; and (5) evaluation.

The Case Manager will report to the Program Manager of the Sasha Bruce Transitional Housing Program. With the support of the program manager and led by the client, creates an Individual Case and Service Plans; all plans must include a goal regarding exit to permanent housing.

The Telephonic Case Manager coordinates resources and creates flexible, cost-effective options for ill or injured individuals on a case-by-case basis to facilitate quality individualized treatment goals, including timely return-to-work if appropriate.

In addition, the LPN Case Manager is responsible for the administration of such direct nursing services, coordination with other medical professionals, and any and all medical oversight as required to fully meet the medical needs of the service recipients in accordance with Department of Intellectual and Developmental Disabilities guidelines.

The Case Manager works closely with ECM and their care team to organize activities necessary to successfully meet requirements established by managed care plans, HEDIS clinical quality measures, and FQHC clinical performance measures.

The Intensive Case Manager will be responsible for assisting individuals and/or families who are homeless and have a chronic illness or physical disability or are high utilizers of the health care system with achieving and maintaining health, mental health, and housing stability.
