- Non-acute care is offered through various affiliates, Summary: Under the direction of the Nurse Manager/Program Director, the Wound Care Coordinator communicates and collaborates closely with physicians, other WCC staff, and other members of the health care team, to provide consistent quality care for patients with wounds.
- Organize and prioritize work to provide health information, discharge planning, and patient care in a safe environment to maximize health in the continuum of care and protect the patient.
- Demonstrates the responsibilities as a professional nurse and case manager through accurate, clear, concise, and timely written and oral communications.
- Guest Relations: Demonstrates regard for the dignity and respect of all patients, family members, visitors and medical center personnel as defined in the philosophy of Vassar Brothers Medical Center.
