Current Hawai‘i Social Work license. Two (2) years of social work experience. Experience working within a collaborative interdisciplinary team. Current driver's license and valid Hawaii auto insurance with short notice access to an automobile to conduct site visits, as required. Current BLS card. Preferred Qualifications: Current Hawai‘i Licensed Clinical Social Work (LCSW) license. Experience in care coordination for patients. Previous social work experience in patient population covered by applicable program. Experience working in clinical projects with interdisciplinary clinical workgroups.
